Why chose a program like Meals on Wheels? Kevin recently sent us this note, and it explains why Meals on Wheels is important to him and his family:

How to apply

Call Lutheran Community Services and request an application for the Meals on Wheels program. Included in the application is information on the nutritional content of each meal, including sodium and calories, so that you may order those meals that fit your dietary needs.

When we receive the application, a member of our staff will contact you to review the application and take your first order. Further orders should be placed by Wednesday of each week for Friday delivery.

You can either order by mail, or order by phone. Seven or more meals must be ordered at a time. If you are a public client, your meals may be free. Private pay clients pay per meal. The volunteer delivering your meals can accept payment.

When ordering Meals on Wheels, you will be able to select from our 22 dinners and 4 breakfasts. We ask that you order a minimum of 7 meals at a time, but you can order more! Our current menu includes:

	Baked Ham with Pineapple		Spaghetti with Meatballs
	Macaroni and cheese		Beef Stew
	Chicken Casserole		Swedish Meatballs
	Baked Chicken and Scalloped Potatoes		Mushroom Omelet
	Roast Turkey with Dressing		Crab Casserole
	Italian Veal with Marinara Sauce		Baked Pollack and Vegetables
	Chicken Fried Steak		Chicken Patty with Mashed Potatoes
	Meatloaf		and more!

To apply for Meals on Wheels

2. Print out the form and fill it out
3. Mail or fax the completed form to us as indicated below.
4. A Meals on Wheels volunteer or staff member will contact you to set up delivery.
